Click RAP Sheet? - Required Response tab

• Is there a RAP sheet? - Choosing Yes reveals Attach RAP Sheet (Required) - panel

• Document Type - Defaults to RAP - RAP Sheet
• Filed By Name - automatically enters your name
• Document Name - Enter document name
• Upload Document - Click + Add Files
• Choose file from Open File dialog box
• Click Open
• Description - enter if needed
