The SLCo Employee Wellness Program provides incentives to reward participants for their engagement in the program.

Medical Forms & Verifications

The SLCo Employee Wellness Program provides points for selected verified medical exams.

Participants can earn points for receiving medical exams and select vaccinations by uploading medical verifications to their personal WellSteps account. Acceptable forms of verification include a practitioner-signed medical form, a doctor's note, or a copy of your explanation of benefits (a statement that your insurance company sends that summarizes the health care services you received). We do NOT accept a screenshot of the appointment reminder or private medical records.

Participants can submit verifications using any of the following methods:

  • Upload verification to personal WellSteps account
  • Email verification to employeewellness@saltlakecounty.gov
  • Courier verification to GC S2-600-4575 with the note "Attn-Employee Wellness"

Earn an additional 50 points by completing the Annual Physical Form with your healthcare practitioner and upload the completed document on to WellSteps or email to Healthy Lifestyles. 

SLCo Employee Wellness Incentives

The SLCo Employee Wellness program rewards participants for investing in their health and wellness.

All SLCo employees and spouses/adult designees who are eligible for benefits are eligible for the annual SLCo Employee Wellness $250 annual cash incentive and the $275 Health Savings Account (HSA) incentive. Annual cash and HSA incentive amounts are based on the level of participation in the SLCo Employee Wellness Program. 

Note: You do not need to utilize county benefits to receive the annual cash incentive, participants just need to be eligible for benefits. Participants who are NOT eligible for benefits can still participate in the SLCo Employee Wellness Program and earn prizes.

To receive an annual cash incentive and Health Savings Account (HSA) incentive, participants must complete an annual physical exam with a healthcare practitioner and submit verification to the SLCo Employee Wellness each year.

The SLCo Employee Wellness annual cash incentives are based on a point system. The more points participants accumulate the higher the annual cash and HSA incentives. Participant incentives will be issued on the SLCo employee's paycheck in December of each year while HSA Incentives will be issued into the participant's Fidelity accounts in January. Please note that annual cash incentives to employees' paychecks are taxable due to IRS Memo 201622031.

Participants begin earning points January 1st and conclude point earnings on October 31st of each year. Incentive amounts are determined by the number of points participants earned between January 1st and October 31st. Participants earn points by completing the following activities:

  • Submit verifications of complete medical exams and vaccinations
  • Engage in Employee Wellness sponsored programs
  • Attend virtual and/or in-person events and workshops
  • Track healthy behaviors in WellSteps
  • Listen to the Employee Wellness Wellcast
  • Participate in Corporate Games
  • Read monthly newsletters and health hub articles

Participants who are enrolled in the SLCo Health Savings Account (HSA) health plan, receive an annual physical from a healthcare practitioner, and is an active SLCo Employee Wellness participant is eligible for an HSA incentive of up to $275 a year.

HSA incentives are deposited into participant's Fidelity accounts in January of each year.

  • Participants switching to a traditional health plan must contact the SLCo Employee Wellness Program by December 1st to receive their HSA incentive before the end of the calendar year.
  • Upcoming retirees must inform the SLCo Employee Wellness program 30 days before their last paycheck to receive HSA incentives based on their current points as long as their previous payment was NOT within the last six months.
  • Employees switching to a non-benefits eligible position or leave the county for reasons other than retirement forfeit their HSA incentive.

New participants can join at any time by emailing us. Participants will have from their start date, to October 31st to earn points to go towards their cash and HSA incentives.

All county employees and their adult designees can participate in the Employee Wellness program. If you are not eligible for benefits, you can earn prizes but are not eligible for the cash and HSA incentives.

 An adult designee can be your significant other to whom you are not married or a family member with whom you share a relationship. In order to qualify, you and your designee must meet certain criteria which includes:

  • Adult designee status for an eligible adult (a child must be over the age of 26 and not disabled) who is:
    • Both unmarried and over age 18
    • Proof of joint expenses
  • If you meet the criteria, you must complete an adult designee affidavit and supply the required financial documentation within 31 days. Affidavit found under Adult Designee link on slco.org/human-resources/benefits/ insurance-benefits/
  • The IRS taxes an “imputed income” for this benefit, which will include an additional cost from your paycheck.
  • The definition of imputed income is the taxable amount an employee pays when adding coverage for an adult
    designee or the children of an adult designee to coverage.
  • Your children up to age 26, regardless of student or marital status (except for life insurance).
    • The child will be removed from coverage at the end of the month they turn 26. COBRA coverage will be offered.
    • Your children of any age with a qualifying disability determined by the health plan. 

Employee Wellness participants use WellSteps to log their points. Log in using your EIN as your username or EIN+1 for spouses and adult designees.

All Employee Wellness cash incentives will be administered to the employee's paycheck in December of each year. Health Savings Account (HSA) incentives are issued to employee's Fidelity accounts in January of each year.

According to the IRS Memo 201622031, an employer may not exclude from an employee's gross income payments of cash rewards for participating in a wellness program. 

The Salt Lake County Employee Wellness program offers a variety of online programs, virtual workshops, virtual classes, and in-person events that span from Clark Planetarium to Herriman Library.
