Targeted Case Management

A new baby can be both exciting and stressful. We can help! If your baby or child is under 4 years of age and on Medicaid, you can have a free consultation and visit with a public health nurse. Your nurse will answer questions about your baby’s care and help you locate the services you qualify for.
