Because of the known health risks associated with tanning, Salt Lake County Health Regulation #25: Tanning Establishments requires establishments that offer indoor tanning to have a tanning operator certified by the health department on site during all hours of operation, among other requirements.
Tanning operator certifications are good for up to 3 years. The application fee is $35.
To become a certified tanning operator, complete the steps below:

Fully complete the Tanning Operator Certification Application below, including the test within the application, and submit the completed PDF to HealthSan@slco.org.
After submitting your completed application, including the test, call 385-468-3835 (Monday through Friday from 8:00 a.m. to 5:00 p.m.) to pay the $35 application fee via Visa or Mastercard.
